Sharing Patient Information

Anyone making healthcare decisions needs access to high quality information: doctors need it to inform their clinical decision making; patients need it when deciding which treatment is best for them; and commissioners need it when making decisions about which services are right for their populations.

Care.data is a new service being developed by NHS England which aims to achieve this ambition by providing timely, accurate information to citizens, clinicians and commissioners about the treatments and care provided by the health service.

Information about you and the care you receive is shared, in a secure system, by healthcare staff to support your treatment and care.

Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researchers and those planning health services, to make sure we provide the best care possible for everyone.

How your information is used and shared is controlled by law and strict rules are in place to protect your privacy.

Your GP Cares Campaign

 
GP surgeries are at the heart of our communities. They are  the cornerstone of the NHS.  But there are problems. GPs are facing increasing challenges which are affecting their ability to deliver services as we all would expect. 
Demand for appointments keeps rising. Older and vulnerable people with complex issues need to spend more time with their GP. The number of people over 65 in the UK reached 10.8 million in 2012 and there are more than 18 million people, with at least one long-term condition such as asthma, arthritis or diabetes.
Pressures on the system are resulting in GPs retiring early and doctors are not choosing general practice as a career. Therefore it is likely that there will be fewer GPs to go around despite the increasing demand and the situation could get worse year-on-year. 
That is why the BMA’s General Practitioners Committee has launched a new campaign, Your GP cares, to highlight some of the pressing issues facing general practice. Your GP and practice team care about the current situation and want to work with patients and government to find solutions and provide a better service. The BMA is calling for long term, sustainable investment in GP services now to:
• Attract, retain and expand the number of GPs
• Expand the number of practice staff
• Improve the premises that GP services are provided from

Named GP for patients over 75

You may be aware that from April all GP practices have been required to provide all their patients aged 75 and over with a named GP who will have overall responsibility for the care and support that our surgery provides to them. Although t his does not prevent patients from seeing any GP of their choice in the practice as they currently do.
Each patient in this age group, should by now have received a letter informing them of their named GP.   No further action is required from patients, but if you have any questions, or wish to discuss this further with us, please contact us at the practice.
 

Commitment to improve patient access to their medical records

Viewing a Summary of Your Patient Record

From 1st April 2015 if you are registered with the practice for online services you will have access to a summary of your Patient Record.

 
To view key details from your Electronic Health Record follow this link and log onto SystmOnline as normal, then click the My Summary Patient Record link which should now appear on the menu on on the Home page.
 
 
The summary will include details of the following:
  • allergies
  • drug sensitivities
  • acute medication (issued in the last 12 months)
  • repeat medication (current and past)
